Please complete the following form in order to enroll your son or daughter in our online Math Tutoring program. Please note that you will receive a “Confirmation of Enrollment” from us before the student can be officially enrolled. 

In addition to the Enrollment Form (below), please also submit the “Policies & Procedures” form when enrolling your son or daughter.

• I/we would like to have the Tuition Fees transferred directly from my/our Bank Account by means of Electronic Funds Transfer (EFT). (Please provide us with an electronic image of a voided cheque, or mail a voided cheque to P.O. Box 21, Caledon Village, ON L7K 3L3)
• I/we will pay the Tuition Fees by Cheque mailed on or by the first day of class of each month, payable to The Math Tutor, P.O. Box 21, Caledon Village, ON L7K 3L3
